Kindly note the below important information related to the digital application forms:

• The form(s) are hosted on the Forms page of our website under the 'Remittance and Foreign Exchange' section. Please click here to download the forms visit www.emiratesislamic.ae/en/personal-banking/personal-banking-forms to access the page.
• Please click on the form to download it to your computer (Desktop or Laptop). The form(s) should now be opened either through Internet Explorer or the latest version of Adobe Acrobat Reader. The form(s) does not support Google Chrome or use of smart phones.
• Once the form is filled (typed out) completely, you will need to click the "Validate and Print" button. The printed and signed document can subsequently be shared with Branch staff to initiate your transaction.
• The form(s) are available as an Editable PDF (Smart PDF) for you to fill in the information directly in the PDF document, rather than the need to print and fill out.
• There are two variants of the form available – Single and Bulk. In case you wish to request for multiple transfers at once, you can use the bulk form to ease capture of your bank accounts details in one go.
• The form(s) should be filled in alpha and numeric combination only, please note that special characters are not allowed.
• Deal Reference Number to be filled only in case you have a special transfer rate which has been arranged with Emirates Islamic.
• The form(s) have been developed by incorporating best industry practices to ensure that the information inputted is legible, complete and authenticated upfront to further improve processing efficiencies and provide you with a better turnaround time.
